About Us

TLC Health Network is a not-for-profit health system that was formed in 2002 as a result of a merger between the former Tri-County Memorial Hospital in Gowanda, NY and Lake Shore Hospital (now Lake Shore Health Care Center) in Irving, NY.

Today, TLC operates facilities in Chautauqua, Cattaraugus and Erie counties including Lake Shore Health Care Center, Gowanda Urgent Care & Medical Center, Forestville Primary Care Center, and outpatient chemical dependency clinics in Cassadaga and Derby. We also operate a home health care agency that operates throughout the tri-county area.
